When trying to decide if liposuction or a tummy tuck is needed, it depends on the problem that is present. If there is excess fat under the skin (fat you can pinch) then liposuction is done to thin the fat. If there is sagging skin and stretched abdominal muscles, either from weight changes or previous pregnancy, then a tummy tuck is done to tighten the abdominal muscles and remove the sagging skin.
If all of the issues are present, excess fat, skin, and stretched muscle, both surgeries are often needed. I do these as separate surgeries because when done at the same time, there is a greater risk of serious wound healing problems, and also not as much fat can be suctioned, leaving more behind which gives a less than optimal result for the tummy tuck.
